What Is the Purpose of Urogynecology?

In today's urogynecology, treating pelvic floor weakness diseases such as lowering the genital organs and urinary or fecal incontinence is the purpose. In addition, there are other urological diseases in women.

What Diseases Are Treated by Urogynecology?

The most common diseases are, on the one hand, urinary incontinence and, on the other hand, complaints due to the lowering or prolapse of organs. Both can be caused by weakening the pelvic floor muscles and the supporting structures.

Incontinence occurs in several forms. The most common ones are stress and urge incontinence.

In stress incontinence, urine is passed involuntarily during stressful situations such as sneezing, coughing, or climbing stairs. This can be the anatomical position of the bladder and urethra, the mechanism of the sphincter, and hormonal factors.

Urge incontinence is the involuntary loss of urine accompanied by a sudden urge to urinate and the feeling that it cannot be held for any time, even though the bladder is not very full. Reasons for this can be urinary tract infections, foreign bodies, tumors, obstructions of the urinary tract, or overreactive bladder muscles.

Lowering of the organs in the pelvis, such as the uterus, bladder, bowel, and vagina, occurs when the connective tissue is weak, for example, due to previous births. This can happen without symptoms and become noticeable through a foreign body sensation in the vagina, protrusion of the corresponding organs, pulling, and pain in the lower abdomen or back. It can lead to voiding disorders of the bladder and intestines and thus to urinary tract infections and pain during sexual intercourse.

What Treatment Methods Are Available to Urogynecologists?

In general, conservative and surgical methods are used. In the case of stress incontinence, targeted pelvic floor exercises are used first, possibly supplemented by local hormone therapy, medication, or weight reduction, to improve. If this does not succeed, surgery can be used either to lift the bladder and urethra (suspension plasty) or to place a tension-free band like a kind of hammock under the urethra to help it close.

Urge incontinence is treated according to its cause and therefore requires careful diagnosis.

Lowering and prolapses can be temporarily treated with special devices called pessaries, which are inserted into the vagina. Surgical methods are also available in this case.

Which Specialists and Clinics Are Specialized in Urogynecology?

Urogynecology is not an individual specialty. However, within the specialty of gynecology, qualifications can be obtained according to the Working Group for Urogynecology and Plastic Pelvic Floor Reconstruction (AGUB) of the German Society for Gynecology and Obstetrics. Urogynecology is offered in outpatient and inpatient settings. In most cases, urogynecology departments with special consultation hours can be found at hospitals for gynecology and obstetrics.
